Facelift

A Facelift may be the answer when the face and neck lack normal skin elasticity and muscle tone, or you have jowls with deep creases from the base of the nose to the corners of the mouth. Facial skin is surgically pulled up and backward in the temple and ear area, sagging muscles are tightened and fat is removed. A neck lift is frequently included with a facelift.

Q. I'm 51 and in the process of launching a new business venture. I've become very aware as I talk with bankers, suppliers, prospective employees, not to mention potential customers, that they often seem to look questioningly at me. I get the feeling that they're questioning my vigor and ability to carry off my plan because my face is well-lined and my eyes are puffy which, I suppose, makes me look much older than I am. I've always thought facelifts were a vanity thing for women but now I'm beginning to feel that it's a subject which I have to approach as a business decision. What can you tell me about facelifts for men?

A. Male facelifts are one of the fastest growing cosmetic surgery procedures today for facial rejuvenation. We believe it is because of the very type of situation you have described. The results of a facelift are a more natural, youthful and rested-looking appearance. If that all adds up to "looking younger," so be it. You mention that your eyes are baggy. Sometimes eyelids need correction to achieve a fresher, rested look. But you indicate your face is quite lined. So a facelift and a blepharoplasty (an eyelid lift) would be the approach for you. These procedures are performed on an outpatient basis under local anesthesia with intravenous sedation or general anesthesia. Sutures are generally removed within one to two weeks. Most swelling and discoloration subsides within a week or two. As a business decision, you should weigh the cost.

Browlift

Endoscopic Browlift, Not Eyelid Surgery, Is Often the Best Way to Improve Sagging Eyelids

Q. When I saw a plastic surgeon about my droopy, sad-looking eyes, he recommended a brow lift. I'm confused. Please describe a brow lift and when it might be appropriate.

A. Many people come to us thinking they need eyelid surgery when what they actually need is a brow lift, which smooths and suspends the forehead, greatly improving the overhang effect in the upper eye region and eliminating that "sad look". As we age, gravity causes our brows to droop and the outer sides of the upper eyelids start to sag.

The brow lift restores forehead balance, smooths skin above the brow and greatly rejuvenates and refreshes the eye region. There are two types of brow lifts. With the traditional open brow lift, incisions are made in the hair above the ears, behind the hairline across the crown of the head. A newer procedure utilizes an Endoscope, a bright light attached to a fiber optic, thin, tubular probe. The advantages to endoscopic surgery are that there are only three to five small concealed incisions behind your hairline, which means much less scarring to hide in the hair.

To see if this forehead plasty might enhance your appearance, look in a mirror and place your fingers above the outer edge of your eyes and your eyebrows. Then gently draw the skin upward to raise the whole brow area. That is the effect you can expect. People who have hooded upper eyelids or bags under the eyes, caused by baggy eyelid skin or puffy fat in the eyelids, are candidates for eyelid surgery. In some cases, a patient may need both a brow lift and eyelid surgery to achieve the desired result. A Board Certified Plastic Surgeon can best advise patients about which procedure or combination of procedures will deliver the best result.

Neck / Chin Contouring

She underwent neck contouring to improve the appearance of her neck and give definition to her jaw line. With the neck contouring operation, fat was removed using liposuction and neck muscles were used to create uplift.

Q. The skin on my neck is getting loose and it's adding years to my appearance. My face is fine but the neck makes it seem older. What's the solution?

A. A "chin tuck", also called a neck lift, would be the likely solution for your problem. Some people only need liposuction, others do best with a neck lift which is part of a combined face and neck lift. For others muscles of the neck are tightened as to create an internal girdle to maintain the youthful angles of the neck. Some people do best with just a chin augmentation, in which a piece of custom-fitted silastic is added to better define the shape of the chin and to stretch the skin of the neck. Finally, there are other individuals best served by a combination of these procedures. These may be performed in the plastic surgeon's surgical facility on healthy men and women into the seventh decade of their lives.
